مقایسه ضخامت لایه های مختلف جدار کولون به روش اندوسونوگرافی در بیماران با کولیت اولسروز و کرون مراجعه کننده به درمانگاه گوارش بیمارستان امام خمینی تهران

بیماری التهابی روده (Inflammatory Bowel Disease=IBD)  یک بیماری روده ای مزمن با واسطه ی سیستم ایمنی بوده که دارای دو زیرگروه اصلی شامل کولیت اولسرو (Ulcerative Colitis=UC) و بیماری کرون   (Crohn’s Disease=CD) می باشد. در دو دهه ی اخیر میزان بروز آن در کشورهای آسیایی و همچنین کشور ایران رو به افزایش بوده است.  روش استاندارد تشخیص IBD بر اساس ترکیبی از علایم بالینی، کولونوسکوپی و پاتولوژی میباشد. افتراق بین UC و CD از نظر انتخاب درمان مدیکال، زمان جراحی و پروگنوز اهمیت دارد. سونوگرافی آندوسکوپیک (Endoscopic sonography=EUS) میتواند لایه­های مختلف جدار دستگاه گوارش را از هم افتراق دهد. با توجه به لزوم افتراق UC و CD از هم و تشخیص سریعتر جهت شروع درمان مناسب و میزان بروز افزایش یافته ی بیماری التهابی روده در کشورمان، لازم است تا بتوان با استفاده از بهترین و ساده ترین روش، این دو بیماری را از هم تمایز دهیم. در این مطالعه ما حساسیت و اختصاصیت EUS را نسبت به روش استاندارد تشخیص IBD (کولونوسکوپی+ بیوپسی و پاتولوژی) و جهت افتراق بیماران UC از CD مقایسه کرده ایم.

Title: Compare the thickness of the various layers of the colon by endosonography in patients with ulcerative colitis and Crohn’s disease referred to Gastroenterology clinic of Imam Khomeini Hospital

Abstract: Inflammatory Bowel Disease (IBD) is a chronic intestinal disease which is mediated by the immune system and has two subtypes of Ulcerative colitis (UC) and Crohn’s disease (CD). In the last two decades, the incidence has been increasing in Asian countries as well as Iran. The standard method of diagnosis of IBD is based on a combination of clinical symptoms, colonoscopy and pathology. Distinguishing between UC and CD is important because of selected medical treatment, surgery, and the prognosis. Endoscopic ultrasound (EUS) can distinguish the layers of the gastrointestinal tract. Because of the need to differentiate between UC and CD and faster diagnosis to start the treatment and increased incidence of inflammatory bowel disease in our country, it is better to differentiate these diseases with the best and simplest method. In this study we compare the sensitivity and specificity of EUS with gold standard method of diagnosis of IBD (colonoscopy, biopsy and pathology) and differentiate the patients with UC from CD.
